Designed for families who need more than basic home care.
Apex Home Nursing Services is the brand. Personal care is provided through Apex Personal Care, PLLC under its Nevada personal care agency license. Separately, Halston and Kayla are available to provide private-duty RN support as individuals, under their own Nevada RN licenses and a separate RN services agreement.
The personal care hourly rate is always $65 per hour. Private-duty RN support is arranged directly with the owners and is separate from Apex Personal Care, PLLC. It is separately documented, separately billed, and not required to receive personal care services.

Personal Care

Private-Duty RN Support
Available separately, arranged directly with Halston Juracka, RN-BSN or Kayla Juracka, RN-BSN under their own Nevada RN licenses and a separate RN services agreement. Support can include an RN home evaluation, a personalized support approach, review of caregiver documentation, family education, medication list review, care recommendations within RN scope, and coordination with your existing providers when authorized.
It is separately documented, separately billed, and not required to receive personal care services. Ask us for details and current rates.

Additional Support
We coordinate the things you do not have time to coordinate.
Home care is rarely the whole picture. Most of our families are juggling pharmacies, equipment vendors, providers, transportation, meal services, and the constant communication that holds it all together. Apex takes that coordination burden off the family.
Coordination Services
Pharmacy refill management and delivery support, medication list review, medical equipment research and ordering, transportation arrangement and confirmation, meal service coordination, specialist visit preparation, records gathering, and communication with existing providers when authorized.
Private RN Support
Available separately with the RN owners: focused RN visits, post-hospital family follow-up, RN family education sessions, advance care planning conversations, family briefings, and RN chronic condition support for diabetes, respiratory conditions, dementia, post-stroke recovery, or post-surgical recovery.
Extended Care Services
Overnight caregiver coverage, 24-hour continuous coverage, holiday coverage, caregiver accompaniment to appointments and outings, and travel accompaniment for clients who travel.
Meal Services
Coordination including options through local Las Vegas physician-developed meal programs tailored to medical dietary needs and preferences.
Coordinated Care
Apex works alongside your existing care. We do not replace it.
Many Apex families are already working with hospice, home health, concierge medicine, or a memory care community. Apex does not compete with these services. We complement them.
Coordination Services

When your loved one needs services Apex is not licensed to provide, we coordinate with the appropriately licensed providers who do.
