For Referral Partners
A Home Care Partner You Can Actually Rely On
From Halston Juracka, RN-BSN, CBIS and Kayla Juracka, RN-BSN, Owners of Apex Home Nursing Services · Las Vegas, Nevada
You already know what a bad referral costs you. The agency that does not call back. The caregiver who no-shows. The family that ends up more confused than before, and somehow it reflects on you. We built Apex to be the opposite of that. We are two registered nurses who speak your language, move at your pace, and treat your reputation as if it were our own, because when you send a family to us, it is.
What you get when you refer to Apex
You are not handing your client to a call center. You are handing them to a registered nurse who owns this company and answers for the outcome. Here is what that means for you, in practical terms.
-
A direct line to an RN owner. You reach Kayla or me, not a gatekeeper. Real clinical judgment on the first call.
-
Same-day response. Referrals do not sit in a queue. We move while it still matters.
-
Full coordination, zero interference. We work alongside existing home health, hospice, and physicians, filling the hours and gaps they cannot, never stepping on them.
-
We document everything. A nurse reviews every visit, and you stay informed without having to chase us.
-
We close the loop. If a problem is ours to solve, we solve it. If it is not, we coordinate with the people who can, so it still gets handled.
-
We make you look good. Your client is supported, your family is calm, and your discharge or your plan holds.
~1 in 5 older adults readmitted within 30 days
A large share of those readmissions are preventable with proper post-discharge support, coordination, and medication management. That is precisely the work we do, which is why your discharge holds when Apex is in the home.

Why each of you sends us clients
These are 2024 to 2025 figures from the CareScout (Genworth) Cost of Care Survey, the most widely cited long-term care pricing study in the country, plus Nevada-specific data. Note that these are medians. Premium settings in Summerlin and Henderson routinely run higher.
Discharge planners & social workers
Readmissions reflect on your team. We provide skilled, supervised support at home, keep families informed, and flag concerns before they become an ER visit. Your discharge holds because a nurse is paying attention.
Memory care & senior living directors
Families want more one-on-one attention than any community can provide. A dedicated caregiver with nurse oversight absorbs that demand, so families feel supported and you can focus on running your community.
Estate planning attorneys
A health crisis is the single greatest threat to the plan you built. We are the infrastructure that prevents those crises, keeping clients stable and situations manageable so the plan stays intact.
Physicians
Your visibility ends when the patient goes home. We maintain a consistent presence between appointments, reconcile medications, and flag what actually warrants your attention, so the anxious calls to your office stop.
Hospice teams
We work alongside hospice without interfering, filling the hours and the practical gaps that covered services cannot, and keeping the family steady so your team can do its work.
Wealth managers
Your high-net-worth clients expect their family handled with discretion and excellence. We are the resource you hand them when an aging parent's health starts to affect the picture, and we make you look brilliant for the introduction.
Confidence that comes from actually doing this
We are not salespeople who learned the vocabulary. Kayla and I are practicing registered nurses. I am a Certified Brain Injury Specialist with a focus on stroke and brain-injury recovery and a nurse educator. We have sat at the bedside, navigated the discharges, argued with the systems, and held the families through the hard parts. When you refer to us, you are referring to peers who know what matters to you, know how to make your clients happy, and know how to make you look good for the recommendation.
And we are reachable. Fast, reliable, and relentless about solving the problem in front of us. If it is outside our scope, we do not shrug. We coordinate with the people who can handle it and we see it through. That reliability is the whole reason a referral relationship is worth having.
The standard we hold, every time
We keep our client list small and exclusive on purpose, and our caregivers are among the best in the state, paid roughly 50 percent above market so they stay and deliver. That is not just better for families. It is better for you, because the family you refer gets our absolute best, and your name is attached to an outcome you can be proud of.
Send us a family and watch what happens. You will get a calmer client, a held discharge, and a partner who picks up the phone. We will earn the next referral by how we handle this one.
