The two registered nurses behind every Apex relationship.

We do not have a call center. When you call Apex, you reach us. When caregiver documentation or family communication shows a meaningful change, we review it and help guide the next step.

We built Apex this way deliberately, and we take on a limited number of families specifically so we can stay personally aware of every one of them.

Halston Juracka, RN-BSN

Halston is a registered nurse with a Bachelor of Science in Nursing and is currently completing his Doctor of Nursing Practice and Family Nurse Practitioner training at the University of Nevada Reno. His clinical experience spans intensive care, stroke and brain injury rehabilitation, acute care, hospice nursing, and outpatient management. He is also a nursing instructor at the College of Southern Nevada.

Halston is Co-Owner and Director of Personal Care Operations for Apex Personal Care, PLLC, operating under the Apex Home Nursing Services brand. Separately, Halston is available to provide private-duty RN support under his individual Nevada RN license and a separate RN services agreement.

Kayla Juracka, RN-BSN

Kayla is a registered nurse with a Bachelor of Science in Nursing. Her clinical experience spans acute care and direct patient care across geriatric and adult populations. She holds the Administrator role for Apex Home Nursing Services and is the regulatory, compliance, and infection prevention lead for the agency.

Kayla is Co-Owner, Administrator of Record, and Director of Administration and Compliance for Apex Personal Care, PLLC, operating under the Apex Home Nursing Services brand. Separately, Kayla is available to provide private-duty RN support under her individual Nevada RN license and a separate RN services agreement.

Our Story

Why we built Apex.

We are a husband and wife team. We have sat with families in the hardest moments of their lives, and we have watched what happens when those families are left to navigate without real guidance. Too often they feel lost. Too often the wrong things slip through. Too often the standard offering is simply not enough.

We built Apex so the families we serve never have to feel that way. A familiar caregiver in the home. A familiar registered nurse reviewing the pattern, guiding the family, and coordinating the next step when something changes. Coordination of everything around them. The quiet confidence of staying informed.

Our Model
Why our agency stays small.

Personal care, at most agencies, is a numbers game. The more clients on the schedule, the better the books look. We built Apex on the opposite premise.

Intentionally limited capacity

We intentionally limit how many families we work with at any time. When we are at capacity, we say so. The families we take on receive the time and attention they are paying for, because we structured the company to make that possible.

Personal awareness of every client

Apex stays small because the RN-owner model only works when Halston and Kayla can remain personally aware of every client relationship. We would rather serve a small number of families exceptionally than a large number of families adequately.

Extended Support

Coordination with outside professionals.

When a family needs support outside Apex's scope, the RN owners may help coordinate with outside physicians, behavioral health providers, pharmacy resources, elder law attorneys, or other professionals selected by the family.

Apex does not employ or control these outside professionals and does not replace their independent judgment.

Apex Home Nursing Services. Personal care provided by Apex Personal Care, PLLC, Nevada Personal Care Agency License Number [INSERT]. Owned and operated in Las Vegas, Nevada. Both owners hold active Nevada registered nurse licenses. Any RN support described on this site is provided by the individual RN owner under that owner's Nevada RN license and a separate RN services agreement, not by Apex Home Nursing Services or Apex Personal Care, PLLC.
