We are Halston and Kayla Juracka, a husband and wife team of registered nurses, and Apex Home Nursing Services is our practice. Not a franchise. No call centers. What Concierge Nursing Actually Is

Concierge nursing means a registered nurse is personally responsible for someone's wellbeing at home. Not visiting a few times and closing the case, the way insurance-based home health works. Not sending a rotating cast of strangers, the way big agencies do. One accountable nursing team, guiding the whole healthcare journey: watching for changes, adjusting the plan, coordinating doctors, and catching problems while they are still small.

Here is the difference in one sentence. A standard agency reacts to what already happened. We anticipate what is about to.

After a combined two decades in ICUs, neuro rehab, hospice, and brain injury care, we have watched the same story hundreds of times: the fall that was preventable, the medication mix-up nobody caught, the slow decline everyone noticed and nobody acted on. Almost every crisis announces itself early, if someone trained is watching. That is what you are actually paying for. Not hours. Vigilance.

What a Concierge Nurse Does

Every Apex client relationship starts with a comprehensive in-home nursing assessment and a written plan built around the person, their conditions, their home, their family, and their goals. From there, depending on the plan: regular hands-on nursing visits, review of every caregiver note from every shift, daily-weight and vital-sign trend monitoring with written escalation thresholds, medication oversight, coordination with physicians and specialists so nothing falls between appointments, family education and honest answers, and early escalation the moment something changes. When our clients see their doctors, their doctors get a briefing. When something shifts at 2am, the family is not guessing alone.

Our caregivers handle daily support, bathing, meals, mobility, companionship, and every one of them is a certified nursing assistant working under our direct nursing oversight. You are never just hiring a caregiver. You are hiring the clinical judgment standing behind them.

What Concierge Nursing Costs in Las Vegas

We publish our prices because families deserve to plan, and because we are proud of what they buy. Nursing visits with us are $200 per hour, delivered by the owners, not per-diem strangers. Caregivers are $65 per hour under nursing oversight. Most families choose a care membership, which bundles caregiver hours, nursing visits, and full coordination: memberships begin at $3,000 biweekly, about $6,500 per month, and range to $9,500 biweekly for our most involved level of care. Every plan is priced after a free in-home assessment, because we will not price care we have not seen.

We are private pay only. No insurance, no Medicare, no Medicaid, which is exactly why we can spend as long as your family needs instead of as long as a billing code allows. Long-term care insurance and VA Aid and Attendance often help families cover this care, and we provide all documentation, and handle the paperwork for our members.

Concierge Nursing vs. Home Health vs. Concierge Medicine

Families searching for concierge nursing in Las Vegas usually find three different things, so here is the honest map. Medicare home health is a wonderful benefit built for one job: a few skilled visits a week, for a short stretch, then it ends. Concierge medicine is a doctor on a membership, excellent for medical decisions, but a physician cannot be in the home watching daily life. Concierge nursing is the layer between them: the daily eyes, the hands-on care, and the clinical brain at the bedside, working alongside your doctor and even alongside home health or hospice, never in place of them. Many of our families keep all three, and we are the ones who make the whole system talk to itself.

Medicare Home Health

A few skilled visits a week, for a short stretch. Then it ends.

Concierge Nursing

The daily eyes, hands-on care, and clinical judgment at the bedside. Ongoing.

Concierge Medicine

A doctor on a membership. Excellent for decisions, not present for daily life.

Who This Is For

Our clients are people who want to stay home, and families who cannot afford to have things missed. Recovering after surgery or a hospital stay. Living with dementia, heart failure, Parkinson's, ALS, or another condition that changes over time. Aging in place in Summerlin, Henderson, and across the Las Vegas valley while the family lives busy lives or lives out of state. Families who tried a standard agency and watched the standard slip.

We keep our census deliberately small. That is not a marketing line, it is the whole design: we would rather serve a handful of families at a standard we would accept for our own parents than serve a hundred at a standard we would not.

Why Nurse-Owned Matters

At most agencies, the owner is a businessperson and the nurse is an employee. At Apex, the owners are the nurses. Every assessment, every plan, every escalation decision is made by the people whose names are on the license, one of us with a decade-plus across ICU and critical care, the other a Certified Brain Injury Specialist who teaches nursing at the College of Southern Nevada and has spent years at the hospice bedside. We built this practice after watching, from inside hospitals and inside great institutions, how many declines were avoidable, and deciding that somebody in this valley should be accountable for preventing them.

That is our mission, plainly: fewer preventable hospitalizations, fewer avoidable declines, and families who get to spend their energy being families, because someone who knows what they are looking at is watching.
