When Standard Isn't Enough.

Premium private-pay personal care, led by registered nurse owners, for families who want more than a standard caregiver agency.

No contracts. No obligation. Just a conversation.

Nevada Licensed Personal Care Agency  ·  RN-Owner Standards  ·  Caregiver in 72 Hours  ·  Private Pay Only

Personal care backed by registered nurses.
Not a staffing agency with a call center.

Your loved one gets a screened, dedicated caregiver for the parts of daily life that have gotten harder: bathing, dressing, grooming, mobility, meals, companionship, and routine. Not a rotating stranger sent to fill a shift, but a consistent person matched to your family and held to a standard most agencies do not keep. Someone who treats your mother or father as a person with a history, not a task list.

That care is provided through Apex Personal Care, PLLC under its Nevada personal care agency license. What makes it different is who stands behind it. Apex is owned and run by two registered nurses, and when your family wants a nurse directly involved, private-duty RN support is available from the owners under their Nevada RN licenses, an experienced nurse who reviews how care is going and helps you make informed decisions.

You choose what fits. Premium caregiving on its own, or caregiving with a nurse beside you. The standard agency cannot offer the second. We were built for it.

Fewer surprises. Fewer hospital trips.
One team that sees the whole picture.

The difference between a small problem and a hospital stay is usually who is paying attention. Changes in appetite, new confusion, a fall that almost happened, the quiet signals that a busy family or a bargain-rate caregiver misses until it is too late. Apex caregivers are trained to notice them and trained to speak up.

When your family adds RN support, an experienced nurse reviews how care is going across the days you are not in the room, watches for patterns, and tells you in plain language what they actually mean. When something needs your doctor's attention, you hear about it early and clearly, not after it becomes a crisis.

Most families wait too long to put real support in place. They tell themselves it can wait until next month, and then a fall or a hospital stay makes the decision for them. The most expensive choice in home care is almost always the slow one. Getting the right people in the home early is what keeps a manageable situation from becoming an emergency.

What We Promise Every Family.

A dedicated caregiver, matched to your loved one.

Not a rotating roster of strangers. The same screened person, chosen for fit and kept consistent.

Nothing quietly slips through.

Care is documented and reviewed, so a small decline or a missed change gets caught and raised with you, not discovered too late.

We pay our caregivers a premium, on purpose.

Higher pay attracts experienced people and lets us choose the rare few who fit. Your money buys the best person in the home, not overhead.

Same-day response, every time.

Call or write and you hear back the same day, from an owner. No call center, no ticket number.

The right fit, or we rematch free.

If the first caregiver is not right within two visits, we rematch. No charge, no friction, no explanation required.

A registered nurse behind it all.

Every caregiver works to standards set by the two RN owners, and private-duty RN support is available directly when you want a nurse more involved.

The caregiver in the home matters more
than anything. We treat it that way.

The caregiver is chosen, not just sent.

We match a specific caregiver to your loved one based on need, personality, and fit, then keep them consistent. That continuity is what makes someone feel safe in their own home.

We take a limited number of families, on purpose.

Standard agencies value quantity over quality, we don't play that numbers game. When we are at capacity, we say so. Your access, your caregiver's attention, and the standard of care all depend on us not stretching ourselves thin.

Nurse-owned standards, with nurse support a phone call away.

Apex care is built on the standards two registered nurses set. And if your family ever wants actual RN involvement, that can be arranged separately and directly with the owners under their own licenses, fully coordinated for you.

What Apex Personal Care includes.

The two registered nurses
behind every Apex relationship.

Nurse

Halston Juracka, RN-BSN.

Co-Owner, Operations

Ten years as a registered nurse across stroke and brain injury rehabilitation, hospice, acute care, clinical education, and quality and compliance. He has guided families through recovery and through the hardest end-of-life decisions, and he built Apex's standards and case management around what he learned doing it. Doctor of Nursing Practice and Family Nurse Practitioner candidate at the University of Nevada Reno, and a nursing instructor at the College of Southern Nevada.

Smiling Healthcare Worker

Kayla Juracka, RN-BSN.

Co-Owner, Administrator

Nine years as a registered nurse across the full range of acute care, including intensive care, emergency medicine, and nursing management. She has run high-acuity units and managed teams under pressure, and she brings that operational discipline to Apex as the agency's administrator, overseeing compliance, infection prevention, and personnel. The person making sure the standard holds, every day, for every family.

Apex is not for everyone. That is the point.

Apex is private pay. We do not bill Medicare, Medicaid, or insurance, and we are not the lowest-cost option in Las Vegas. If price is the deciding factor, we are probably not your agency, and we will tell you so honestly.

But if you have been quietly holding the people you love to a higher standard than what you keep being offered, that instinct is not extravagance. Wanting excellent care, for a parent, a spouse, a disabled family member, or for yourself, is the right instinct, and Apex was built for people who refuse to settle on it.

We built it for situations like these.

The family managing a loved one's care from across the country, lying awake wondering if anyone is really paying attention on the days they cannot be there.

The spouse caring for a partner through dementia or recovery, carrying alone what used to be carried by two.

The person coming home after surgery or a hospital stay who wants real support during recovery, not a rotating cast of strangers who never learn their routine.

The family caring for a disabled adult who deserves consistency, dignity, and people who actually know them.

The family already working with hospice, home health, a concierge physician, or a memory care community, who needs dedicated private support in the hours those services cannot cover.

If you recognize your situation here, the first call will tell you quickly whether we are the right fit. And if we are not, we will point you toward who is.

Why Apex costs more
than standard home care.

If you have called around, you already know we will not be the cheapest. We made that choice deliberately, and we would like to tell you exactly what it pays for.

It pays our caregivers well above the going rate, because the people who are truly good at this have their pick of families, and we want them choosing yours. It pays for a standard set by two registered nurse owners rather than a scheduling desk. It pays for an owner who answers the phone, documentation you can actually see, and a caregiver chosen for your loved one rather than assigned by whoever was free that morning.

None of that is the least expensive way to run a home care company. It is the way we would want care delivered to our own parents, and it is the only way we were willing to do it. Families who have tried the alternative tend to understand the difference quickly.

We do not hide our prices.

We told you we are not the cheapest. Here is exactly what it costs.

Personal Care

$65 / hour

Premium nonmedical caregiver support, provided through Apex Personal Care, PLLC under its Nevada personal care agency license. Registered Nurse support, coordinated separately

Most families want more than a caregiver. They want an experienced nurse watching the whole picture. Separately from your personal care, private-duty RN support is available directly from Halston Juracka, RN-BSN and Kayla Juracka, RN-BSN, under their own Nevada RN licenses, at the level of involvement your family wants, from periodic check-ins to close, ongoing nurse involvement.

RN support is recommended for every Apex family because it is what makes the Apex experience what it is. It is not required to receive personal care. It is arranged, billed, and documented separately by the RN owners.

To discuss the right level of RN involvement for your situation, just ask when you call.

No long-term contracts. Private pay only. Call or text us and we will walk you through exactly what your situation needs.

Want to understand this before you call?
Start here.

Choosing care is one of the most important decisions a family makes, and the most expensive mistakes usually come from waiting too long or not knowing the real options. These are the things we wish every family understood before they had to decide.

The Real Cost of Waiting

The financial and human math families rarely run until it is too late. What a single fall can lead to, the true monthly cost of memory care, assisted living, skilled nursing, and rehab in Las Vegas, and how early in-home support compares to the cost of the crisis it prevents.

What to Ask Before You Hire Any Home Care Agency

A plain-language checklist of the questions that separate a premium agency from a staffing service. Who answers the phone, is there clinical oversight, dedicated caregiver or rotating roster, what is the screening process, what happens when the fit is wrong.

The Five Signs It's Time for More Support at Home

The early warning signs families miss or rationalize: weight loss, medication confusion, a near-fall, withdrawal, the caregiver-spouse wearing down. Helps you recognize that the moment is now, not next month.

Nurse and Patient on Wheelchair

You do not have to have it
figured out before you call.

That is what the first conversation is for. Call or text either of us directly. We respond the same day.
